Please remember that when you fill out a check request form, it must be filled out completely with original receipts attached and 2 PTA committee member signatures. All money needs should be given to the Treasurer directly and should NOT be left in the cabinet or the front office.

PTA Membership Sign Up Form
If you would prefer to print out your membership form, please print this form. Send it in with your student (with payment) who will turn it into their teacher. If it is during the first week of school, you can bring it to the membership table in the front lobby.
